A Schedule of Arrearages is a form that is filed with the court regarding previous court ordered monthly payments that are past due because of non-payment. The person filing the form is looking to collect the payments.

What information is included on the Schedule of Arrearages form?

The Schedule of Arrearages form is a two page form. The first page contains information on the involved parties, the case number, dates and who the owing party is. The second page contains five columns to be filled out by the party that is owed the monthly payments.

What information is included on the Schedule of Arrearages form?

These five columns are as follows:
